🚀 How to use this template

Responsible

🛠 Who is responsible for completing the task or activity? Limited to a single individual or a team. Responsibility should be assigned by the individual Accountable for the task/activity.

Accountable

📌 Regardless of who does the work, one person must ultimately be accountable for the delivery.

This individual may also be the one responsible for the work or they may delegate.

Consulted

📣 They are not responsible for the work, and not accountable, but their opinions and input are needed.

These are the folks that should have a say in the task/activity being completed.

Informed

✏️ Keep these people in the loop. They aren’t required to contribute anything but should be kept up to speed on activity and outcomes.

Warning: This can easily become a trap of “everyone”. Give this column the same due diligence as the others when considering who needs to be involved.

Bonus: Out of the Loop

🤷🏻‍♂️ Times will come when there are many projects on the go. Identifying who can voluntarily be kept out of the loop is a sign of effective communication and trust within an organization.

What Activities to Include

🤏🏻 Keep the focus narrow but not too narrow. Each task/activity should be specific enough to assign an accountable owner. But not so narrow that you’re repeating the same function over and over.

Example:
